Our role in the isp process

We are deeply engaged in the Individual Support Plan (ISP) process, partnering with ISCs and each person’s Circle of Support to ensure holistic, meaningful, and goal-oriented service delivery.

Assessment: We evaluate each individual’s needs, strengths, challenges, and aspirations through respectful consultation.

Integration: Goals are translated into measurable outcomes within the ISP, emphasizing independence and community inclusion.

Monitoring: Regular reviews and progress tracking ensure services meet expectations and adapt to changing needs.

Resolution: Our transparent complaint and appeals system empowers individuals to express concerns and shape their supports.

We believe in the power of partnership—with ISCs, families, and providers—to achieve the best outcomes. Open communication and shared goals are the foundation of our success.
